Presenter Final Report Forms

Failure to submit reports in a timely manner may jeopardize funding.

Fee Support and New Directions Grants

There are two different ways to submit a Presenter  Fee-Support or New Directions Final Report Form, depending upon how you submitted your application.

1) If you applied using eGrant (February 2007 deadline or later), then you must access your final report form through the eGrant system. Your report form will appear on the list of applications/reports after you log in.

2) If you applied using the older Word form or the paper application, you may download a printable PDF form.

Presenter Travel Grants

Grantees must submit a final report form for the project and a reimbursement form with appropriate documentation and receipts.  Access a writable Word form or printable PDF form of the final report form here.  Click here for a copy of the Compliance Agreement.

 

IMPORTANT: In order to save a blank or completed report using the writable Word form, select "File" from the top menu of your browser or word processing application, and then select "Save As". Please open and save a new version of this form for each new project, as the form may change over time - do not use an old saved version.

Artist Final Report Form

The Artist Final Report Form (for both Technical Assistance and Strategic Opportunity) is available as a writable Word form or a printable PDF. If using the Word form, after you have opened and saved this document to your computer, you will be able to type directly into it.
